However, the management makes it compulsory that any new employee of the company must undergo two months course on safety and accident prevention which are always done by the Manpower Training School. Insurance Policy for workers, First Aid Box, application of appropriate safety protecting materials as well as electronic gadgets installed at the company’s premises and workshops has enable every employee enjoy our safety policy.

WORD PROCESSING
Microsoft Word for windows 2000 is the basic program used by TEMDASEM GLOBAL RESOURCES NIGERIA LIMITED for word processing. Write 6.0 is also available.
ACCOUNTING
The accounting system, Open Systems Accounting Software, is a stand alone package that is modularized to permit its use a variety of configurations. The basic accounting functions of TEMDASEM GLOBAL RESOURCES NIGERIA LIMITED are served by this package where the General Ledger is the hub module.
PROJECT MANAGEMENT
The Project Management programs utilized by TEMDASEM GLOBAL RESOURCES NIGERIA LIMITED include Windows based Microsoft DOS based Harvard Project Manager. All the programs are similar with regard to critical path analysis, float time, resource tracking, calendar modification, etc. In order to achieve their objective it is the policy of TEMDASEM GLOBAL RESOURCES NIGERIA LIMITED to establish and maintain an elective and efficient quality system, planned and developed in conjunction with other management functions. The system shall comply with international standards.
2. Quality Policy Statement
The Quality Plan procedures are in line with internationally recognized standards such as BS, IEC, IEE regulations etc. Additional provisions are also made for the special requirements/ standards adopted required by our various Clients.
The main object of this Quality Policy is to achieve an economic, safe, and functional project quality system. This system should be maintained and improved upon with the objective to ensure that the services provided by the company are according to the internationally accepted standards, rules and regulations.
This objective will be achieved by the following procedures and as described in the manual of the Company, and by a continuous monitoring/weighing of the targets against the actual quality. The project Quality Plan describes procedures concerning quality control to be followed for the review of project construction and commissioning. The basis for the project Quality Plan is laid down in the corporate Quality manual of the company.
Management is totally committed to obtaining the highest professional standards and seek the cooperation of all employees in order to achieve these goals. . Needles to say, the quality of the services of our sub-contractors are also our responsibility.
3. Project Organization
A project organization is established depending on the contract demands. The responsibility of the Project Management group is to organize the work in all its aspects in close relation with the Client Project group and the Company Quality organization. The organization consists of but is not limited to the following activities.

3.2 Managing Director
The Managing Director has the day today, overall responsibility for the organization, and reports to the Board. He is responsible for the management and execution of all Construction Works in the Organization and all other related matters in running the Organization as a whole.
He will enforce, in full the requirements of all Contracts in terms of safety, quality, efficiency and schedule. He represents in all matters related to the project(s). He establishes the general site organization. He may delegate the performance of any duties to those who report directly to him and who are qualified to perform such duties.
The following responsibilities, however, cannot be delegated:-
3.2.1 Instructions
1. Liaison with the Client for general project management.
2. Initiate a project organization
3. Co-ordinate project team members
4. Control approved detailed procedures, issued by the Quality Engineer (Project)
5. Contribute to the annual managerial review of the Quality Programme documentation.
6. Contribute to and join in procedures as described in the Quality programme and specific project procedures.
7. Initiate corrective actions related to the project.
8. Enforce full implementation of the established (Project) Quality Plan (Manual).
9. Stimulating the Project personnel in quality awareness by facilitating skill and training, and providing unambiguous and clear instructions.
<3.3 Projects Manager
The Projects Manager is under supervision of the Managing Director and responsible for the actual construction of the works. He is responsible for the performance of construction operations in conformance with project requirements, using qualified personnel and procedures, and assuring that the required level of quality of workmanship is maintained throughout the construction process.
He co-ordinates and supervises the performance of the works of the sub-contractors (if any) and ascertains that the sub-contractors assigned work is in accordance with approved procedures and design requirements using personnel qualified to implement these procedures when applicable.
Besides his function, he is also instructed with respect to the Project Quality Plan to:
1. Initiate the development, establishment and maintenance of the Quality Plan by the following actions:
2. Facilitate the development of the Quality Plan.
3. Implement a Quality organization.
4. Participate in the auditing of the implementation of the Quality plan, through the Quality Engineer
5. Initiate corrective action
6. Contribute to the annual managerial review of the Quality Programme documentation.
7. Stimulate the company personnel in quality awareness by facilitating skill and training, as well as by authorisation of consistent instructions.
3.4 Project ManagerM
The Project Managers work under supervision of the Projects Manager.
They co-ordinate construction activities in their respective projects and assure conformance with project requirements.
They supervise and co-ordinate the performance of the works, including the works of the sub-contractors under their units. They report to the Projects Manager and ensure that all works are executed in accordance with approved procedures and design requirements, as follows:
1. Stimulate their units personnel and sub-contractors in quality awareness, through training2. Instruct supervisors with respect to Project Quality Plan
3. Assist with the implementation of Quality Organisation.
4. They participate in proposals to keep the progress of activities on schedule
3.5 Senior Engineers
The senior engineers are under the supervision of their respective Project Manager
1. They develop and are responsible for the implementation of engineering procedures and instructions necessary for the performance of engineering functions in accordance with the project requirements
2. They provide the technical support to their Project Manager to ensure that construction is in accordance with the project requirements.
3. They are responsible for the supplementary project specifications, being the translator of the project specifications and requirements into construction drawings, specifications procedures instructions, and to correctly perform procurement, fabrication and installation of the assigned construction.
4. They assure that As-Built data are registered and filled for final handover to Client and shall provide the necessary data for progress of work reports.
5. They provide engineering progress reports on regular basis identifying all standards, specifications and required construction permits. This engineering report will also contain a list and planning for the production of drawing such as route maps, crossings, facilities requisitions etc. to facilitate progress control.
6. They report findings, which may affect the quality of products and services, to the direct Supervisor and communicate with the responsible inspection personnel.
3.6 Engineers
The engineers under the supervision of the senior engineer(s) are responsible for the engineering support.
1. They develop and are responsible for the implementation of engineering procedures and their instructions necessary to perform engineering functions in accordance with the project(s) requirements.
2. They provide the technical support to the supervisor to ensure that construction is in line with project requirements and assures that As-Built data are registered.
3. They provide the necessary data for work progress reports.
3.7 Quality Engineer
The quality engineer is under the supervision of the Project Manager.
1. He initiates and/or prepares and supervises the implementation of the quality plan and takes those actions necessary to assure the quality, safety and integrity of products and services provided by the organisation.
2. He is to supervise the Project Quality System within the project organisation, comprising but not limited to the requirements for material identification and inspection, construction and testing, verification and certification, handling, transport and shipping and certification of craft personnel such as masons, carpenters and inspectors, as well as equipment for special processes.
3. He evaluates the extent and adequacy of existing basic procedures within the project organisation and advises the Managing Director about the need for development of new procedures.
4. He contributes to the establishment of necessary new procedure in close co-operation with the Projects Manager, the management of the project organisation and the Managing Director.
5. He distributes and keeps control of any particular Quality programme document.
6. He ensures that all required quality documentation of projects are submitted to the Client through communication lines detailed as follows:
7. He provides Quality Plans for projects in compliance with the contract requirements.
8. He provides inspection instructions and acceptance criteria, by way of Quality detailed procedures.
9. He identifies and reports possible quality problems to the Projects Manager.
10. He initiates solution of such problems.
11. His co-ordinates quality related activities on possible secondary sites, if pertaining to the responsibility of the subject project team.
12. He provides status reports to the Projects Manager.
13. He ensures proper inclusion of all relevant quality requirements in Purchase Orders and Sub-contract documents.
14. He prepares, executes or co-ordinates and reports systematic audits in the project organisation, suppliers and sub-contractors organisation to ensure that approved procedures are adhered to.
3.8 Materials Controller
The material controller is under direct supervision of the Project Manager and is responsible for the project material control.
He assures that material data are registered and filed.
He provides the necessary data for the progress reports.
His duties are as follows:
1. Filling of material related data
2. Update the material registration system
3. Provide material reports
4. Detect shortfalls in time, so that the operations can be executed according to approved scheduled/planning.
5. Provide procedures for material take over and report findings which may affect the quality of the project to the supervisor.
3.9 Supervisors
1. Supervisor all organizational and technical activities within the work units
2. Implement controls and initiate actions necessary to assure the quality, safety and integrity as well as contractual delivery time of the project works, made of as follows:
3. Supervise the implementation of authorized and valid instructions described in Quality procedures and subordinated documents, which are provided by his direct supervisor.
4. Perform authorized corrective actions.
5. Report findings, which may affect the quality of products and services, to the direct supervisor and communicate with the Quality Engineer.
4. Document Control System
4.1 Document Identification
The Document Control System has been established to record the movement of documents to ensure that they are properly filled, traceable and processed in a timely manner. Software will be used to create pre-formatted document templates that will allow the personnel available to produce consistently high quality correspondence within minimum time.
4.2 Document Naming/Numbering
The Document Naming/Numbering system is designed to identify the parties/companies involved, document type, sequential numbering and year.
4.3 Documents Stamping
4.3.1 Documents for Comment
Documents with this stamp are submitted to the Client for comment.
After return the comments will be processed
4.3.2 Documents for Review
After processing the comments the documents will be submitted for review.
The outgoing documents carry the “FOR REVIEW”
4.3.3 Documents Approved For Construction
After implementation of the comments of design review, the documents will be submitted to the Client for Approval for Construction
The outgoing documents carry the stamp
“APPROVED FOR CONSTRUCTION”
Only documents which are approved by Client will be used for construction purposes
4.3.4 Documents for As-Built
After construction, the approved for construction documents will be adapted to the actual construction data.
After adapting, these documents will be stamped
“AS-BUILT”
4.3.5 Documents for Void
Documents will be void when they are either replaced by a later revision or when they are rejected, by the Client or Contractor, completely.
All documents which are no longer valid, will be marked with stamp “VOID” and will not be used any More for extracting information for construction purposes.
The Senior Engineer is responsible for marking documents and the distribution of this information.
4.4 Design Documents
All drawings will be identified in accordance with the agreed numbering. The drawing dimension will be formatted on A1 or A3 or A4 paper size
The drawings will be compiled and will contain all relevant basic information such as
1. document number
2. title
3. dimension
4. sale
5. draftsman’s name
6. revision
7. check/approval
The design documents will be distributed to all concerned. The receiver of such documents is responsible for appropriate filing, storing and treatment of the documents. The Admin Officer is responsible for recording a distribution list. H is also responsible for temporary distribution. For external distribution the drawings will be attached to a transmittal.
Final approval of the different stages of design will be done by the Client; however the internal checking of correctness before submitting to the Client will be done by a person other than the draftsman, with at least the same experience and level of education as the draftsman.
The drawing will be stamped “ISSUED FOR COMMENT” and will be signed for checking by the Project Manager or the Units Heads. Te same procedure will be followed for up-dating.
The Managing Director is responsible for releasing the documents.
4.5 Material and Equipment Certification
All procure project materials/equipment shall be covered by certificates. Similarly, all certificates for materials and equipment supplied by Client(s) will be delivered for inclusion in the final As-Built report (if required).
4.6 Data Sheets
The Data Sheets must contain all relevant information for construction and requisitioning. The data will be related to and refer to specific drawing number(s) and/or specification(s). The Data Sheets from an integral part of the design documents.
The Admin Officer is responsible for setting up an appropriate filing system.
The Quality Engineer is responsible for verifying this system.
4.7 Equipment Drawings
The equipment drawings will be treated similar to design documents, they will be (re)numbered and (re)titled.
Client is responsible for submitting all relevant vendor drawings to the organisation.
The Project Manager is responsible for the distribution to all concerned.
4.8 Vendor Data
For all construction materials and equipment supplied by the Client, corresponding vendor data will be supplied.
4.9 Monitoring
When non-conformance occurs, with respect to design, procedures, control of project execution, etc which may seriously affect the safety, quality or environmental aspects of the project, Client will be notified by a written report.
The report will comprise at least
1. Description of deviation
2. Cause
3. Effect of the deviation
4. Remedy
It is responsibility of the Quality Engineer that the advice is according to the applicable standards and specifications. The Client will clarify his intention by written report or letter which clearly expresses the actions to be taken by the organisation. In case corrective actions have to be taken, the Project Manager is responsible for authorisation of the execution and that all concerned are notified. Quality Engineer is responsible for verifying the implementation of the corrective actions.
4.10 Agenda
The agenda for a meeting should be submitted in time to all participants, the agenda will contain at least the following information.
1. Project title
2. Contract number – sequential number
3. Venue
4. Date and time of meeting
5. List of participants (Client, contractor, others).
6. Date of issue
7. Introduction
8. List of items for discussion
9. round-up
10. Closure
5. Work Procedures
To assure the quality of the services provided by TEMDASEM GLOBAL RESOURCES NIGERIA LIMITED, a detailed work procedure will be prepared for each activity which sets out methods by which the activity will be carried out in accordance with contractual technical, HSE and quality requirements, specifications and industry standards.
These detailed work procedures are listed in the Quality Manual Method statements. All matters concerning safety are described in the HSE Manual. All quality control requirements and plans, preformed quality records, scheduled of quality auditing, monitoring and control of no-conformance, corrective actions and hazard analysis, applicable to each activity, will form part of the work procedures.
1. Foreword
This Safety Policy and Working Procedure is a basic guide to establish safe working conditions on all sites under control of TEMDASEM GLOBAL RESOURCES NIGERIA LIMITED covers the most important safely procedures and techniques, but does not replace common sense when the situation so requires.
It is TEMDASEM GLOBAL RESOURCES NIGERIA LIMITED policy to maintain a safe and healthy place of work, but this can only be accomplished with the full co-operation from everyone involved in the construction operations.
Note: It should be clearly understood by all, that failure to comply with the Safely Instructions could result in disciplinary action.
2. Health and Safety Policy Statement
Policy Statement
It is TEMDASEM GLOBAL RESOURCES NIGERIA LIMITED policy to take every reasonable and practical step to first provide and maintain for every employee a safe and healthy work environment and secondly ensure that the health and safety are not jeopardized as the result of its operations.
TEMDASEM GLOBAL RESOURCES NIGERIA LIMITED encourages all employees to work safely and supervise as necessary.
TEMDASEM GLOBAL RESOURCES NIGERIA LIMITED provides such health and safety services necessary to fulfil this policy.
TEMDASEM GLOBAL RESOURCES NIGERIA LIMITED has instituted and maintained the necessary arrangements to ensure that this policy is implemented and monitored.
3. Requirements before Site Mobilization
3.1 Identification Requirements
All personnel entering sites should carry a valid means of identification such as a current identity paper.
3.2 Safety Training Requirements
Personnel shall have undergone an appropriate safety training program to conform to the company rules and the locally valid relevant national legislation.
Personnel entering site for the first time shall be given a safety briefing at an appropriate location.
3.3 Health Requirements
Personnel should:
• Ensure that all medical check-ups are up-to-date. Be in good health.
• Report any drugs or medicines prescribed by a doctor to the site supervisor/project Engineer in charge.
• Report to the immediate Supervisor if you feel unwell at any time.
• Maintain standards of personal hygiene at all times to safeguard the good health of yourself and others.
Personnel should note:
• Be under the influence of alcohol or drugs while on duty
• Be suffering from a communicable disease
• Have any condition which may impair their judg3ment or ability to carry out their duties
4. Safe Working
4.1 Attitude
General awareness of plant, equipment and materials used, as well as the area condition in which the work has to be executed are the key of safe working. All personnel should be aware of all things that may effect the execution of their work.
4.2 Housekeeping
Good housekeeping is essential due to the type if equipment and material used on sites
The main requirements are to:
• Keep all things clean and tidy,
• Put waste in the special containers and do not throw anything on sites,
• Store paint and chemicals in appropriated locations, e.g. paint locker,
• Store tools, equipment and materials in proper areas
• Keep passage ways clear
• Clean up spills
4.3 Personnel Protective Equipment
Personnel protective equipment should be used when appropriate. Hard hats should be used at all times, especially when handling loads with a crane or where there exists a risk of falling objects. Safety Shoes should be worn at all times.
Other equipment to be used when appropriate and available on site is: Safety goggles, ear-protection, face shields, dust masks and filters, breathing apparatus gloves and safety belts.
4.5 Tools
Tools used should be maintained in good working order. In case of doubt or when damaged they should be handed in for checking and repair. Do not improvise; use the right tool for the job. Never use equipment with guards removed. Never insert hands or articles through a guard. Wiring of electrical tools should be checked regularly for damaged.
4.6 Equipment
Ensure that operation of equipment is carried out by qualified personnel and is operated in a safe manner within its designated limits. Maintenance and repair of equipment should only be performed by qualified persons.
The right equipment should be used for the job. Equipment should always be used within its operation limits. In case of any doubt, a supervisor shall be contacted. Equipment showing any sign of shortcomings should be reported and not be used until appropriate maintenance has been applied.
5. Personnel Safety Regulations
5.1 Smoking
“No smoking” signs clearly mark all areas where smoking is prohibited. Smoking, naked light, matches, lighters or any other apparatus which can cause ignition is forbidden in every part of a restricted area.
5.2 Alcohol and Drugs
Alcohol and drugs tend to slow down reflexes and decrease the sense of responsibility, therefore the consumption of any and working under its influence is strictly prohibited during working hours except for medicines prescribed by a doctor. The presence of alcohol or drugs in the labour-yard is prohibited.
5.3 Clothing
Do not wear loose, baggy clothing, neck ties or jewellery, which might become entangled in equipment. Employees must wear appropriate clothing at all times while working on a job. Hot water, steam or compressed air must not be used to clean clothing, boots or shoes while they are being worn. Petroleum spirit or other low flash-point cleaning fluids must not be used to clean clothing.
5.4 Hazardous Conditions
It is the responsibility of every employee to assist in the detection and reporting of unsafe conditions and situations. All hazards must be reported immediately to your supervisor and the cause eliminated as soon as possible. If any area becomes hazardous as a result of combustible gas or vaporizing liquids, all work mist be stopped immediately until the condition is brought under control.
5.5 Signs and Notices
Observe all warning signs, notices and instructions at all times. Individuals should check the bulletin board each day and take notice of newly posted information. Safety signs and notices are posted in your benefit.
5.6 Injuries
Report all injuries, whether serious or minor. All injuries must be given first-aid treatment and reported to your immediate supervisor as soon as possible.
5.7 Accident Reporting
The primary purpose of accident reporting and investigation is to discover hazardous conditions and practices so that accidents from similar causes
can be prevented. The supervisor involved shall personally investigate every accident in order to eliminate hazardous conditions or unsafe practices.
All accidents and emergencies shall be notified to the client’s representative. Accidents and emergencies of a serious nature require a written report.
5.8 First Aid and Illness
5.8.1 General
On site there will be a staff member trained in first aid to treat illness.
5.8.2 First Aid
All personnel should attend a general safety introduction course. The following items are of importance when aiding a victim of an accident.
1. Take care of your own safety and of those around you;
2. Make the area around the victim safe if possible (e.g. switch off electricity) but do not otherwise disturb the area;
3. call for a first aid trained staff member as soon as possible;
4. Do not move the victim unless he is in further danger;
5. Try to stop serious bleeding and apply first aid as appropriate;
6. In order to avoid a situation of shock, make the victim as comfortable as possible.
5.8.3 Illness
Common sickness and illness which do not result in considerable loss of time shall be treated on site.
6. Working Procedures
6.1 Working Overhead
Ensure adjacent areas are marked and barricaded to prevent injury from falling objects to persons below. Scaffolding should be provided with hand rails. Access ladders should be securely tied to the scaffolding or to a fixed structure. Personnel working at height (more than two metres) shall wear safety belt, unless it should be deemed safer to be without.
Heavy or bulky tools and materials should be raised and lowered by line and not carried up or down ladders.
6.2 Hoisting And Cranes
Only authorized operators shall operate hoisting equipment/cranes and one person will be designated to give instructions to this operator. Radio communication or hand signals will be used to give these instructions,
Chain, slings and shackles shall be inspected and certified for the intended lift. This shall be supervised by a competent supervisor to ensure correct hooking on of slings and installation of tugger or tag lines. All personnel shall stay well clear of the load to be lifted or landed. Warn others who come too close. Riding on a load is strictly prohibited.
7. Fire Prevention and Fire Fighting
Fire is an ever-present risk and PREVENTION is better than cure. Most fires can be prevented by adhering to the following simple rules:
• the observance of “no smoking” areas,
• smoking shall not be allowed in certain areas i.e. flammable storage areas, and other areas specified by the Company;
• being fire conscious and observing relevant regulations;
• correct storage of flammable materials;
• a specific area or areas (as required) shall be set aside for storage of hydrocarbon flammable, paint, solvents, etc;
• The use of some flammable solvents to clean up oil spills on machine parts is strictly prohibited. Generally an acceptable solvent must have a minimum flash-point of 43 oC (110 oF);
• solvents used in cleaning must be kept in clearly identified cans;
• spilt hydrocarbons shall be cleaned up immediately;
• cleanliness and tidiness, especially when disposing of cigarettes ends, matches, hot materials and oily rags;
• one man shall be designated to gauge fuel tanks when filling;
• the correct use and maintenance of electricity equipment;
• oily rags for waste shall be stored in metal cans or barrels;
• All lights on the end of a drop cord must be of the guarded type and double isolated. Explosion proof lights are required in areas where there may be a release flammable vapours;
• all portable electrical tools must be double isolated;
• all torches and portable battery powered lamps must be approved for hazardous area use;
• compressed gas cylinders must not be used without a pressure reducing regulator;
• All compressed gas cylinders must have their contents clearly identified, be stored upright and fastened by a chain. All cylinders must have valve caps in place when being moved;
• The use of any type of flame for locating gas leaks is prohibited. Use water, soap and water, or a portable gas detector.
12. Handling of Materials & Movement of Equipment
12.1 Handling of Materials
12.1.1 General
When handling materials both with and without mechanical assistance employees must be alert at all times to prevent accident and injuries.
Under no circumstances shall the safe working load (SWL) of any equipment be exceeded. All lifting tools or materials shall be stamped or certified with this SWL in manufacture.
12.1.2 Cranes
Mechanical handling of materials must be done only by those persons who have full knowledge of the equipment and have been authorized to use it. Personnel must keep away from objects being raised or lowered and must keep clear of suspended loads.
Loads shall never be left suspended on an unattended crane.
The operators shall never start the machine movement until the lead man is within this range of vision, or in radio contact and the signal is understood. Where several individuals are involved, the operator shall obey the signal of the lead man only. The only exception shall be in the case of an emergency, when the operator can receive the Emergency Stop Signal from anyone.
When radio communication is not feasible then standard hand signals shall be used in all crane lifting operations.
12.1.3 Chains, slings and wire ropes
Chains, slings and wire ropes should periodically be inspected and kept in good condition.
12.1.4 Overhead Loads
Never pass beneath or work under a suspended load. Use tag lines or guide poles when necessary to guide suspended materials or equipment
Always wear a safety hat in the vicinity of lifting activities.
12.2 Movement of Equipment
Permission must be taken before moving any equipment. Location and destination mist be designated. Only qualified operators are authorized to drive and a special permission should be obtained if movement is after working hours.
13. Scaffolding, Exposed & Elevated Areas
13.1 Scaffolding
Scaffolding shall be erected only by men sufficiently experienced and qualified. Scaffolds must be adequately braced, completely planked and never overloaded. The platform shall be lifted with adequate guardrails and well fixed ladders.
13.2 Ladders
Do not use defective ladders or scaffolds. Makeshift repairs shall not be permitted. Secure portable ladders to prevent slipping or falling by tying the ladder to a secure structure.
Always face the ladder and use both hands when climbing or descending. Never carry heavy and bulky tools and materials up or down a ladder. Such articles can be raised and lowered by means of hand lines.
Do not place a ladder in front of a door without making some provision to prevent the door from being opened. Do not splice ladders together. It is dangerous to lean the body beyond the ladder rails to gain addition reach. Move the ladder as the work requires. Do not use metal ladders when working on electrical equipment.
13.3 Walkways and Stairways
Walkways and Stairways must be well constructed and maintained in good condition. Standard handrails and midribs must be provided for both walkways and stairways of over three steps in height. Oily, wet or outside stairways or walkways should be constructed of punched sheeting, expanded metal or similar non-skid materials. Apply sand or salt to walking surface when icy conditions exist. Walkways and stairways must be free of loose materials.
If manhole covers, pit covers, trench plate covers, platform sections, handrails, etc are removed, and the openings shall be properly guarded and replaced when the work is finished.
13.4 Elevated and Exposed Work Locations
A safety belt shall be worn when working at height of more than 2 meters or in other exposed conditions unless a specific procedure precludes its use. When permanent handrails have to be removed from elevated platforms, rope or wire handrails should be fitted in their place.
14. Confined Spaces
14.1 Confined Working Locations
Usually confined working locations have the following properties:
• difficult to ventilate;
• contents relatively small so that escaping gasses fill this space quickly with dangerous concentration for men working in this atmosphere
• access to a confined space is usually difficult so that it is difficult to evacuate in case of an accident
Before and during the work in confined spaces ventilation must be maintained, especially in case of welding.
In confined spaces welding must be with direct current only
There shall always be an extra man outside the confined space who can observe the person working inside. The observation will be his only job. In case of hazard, he will ask for immediate help.
In some cases it will be necessary to use an air or oxygen mask
Working in confined space must be pre-authorized.
15. Electrical Works
15.1 General
The following rules are issued to ensure the safety of all personnel engaged in the operation and/or maintenance of electrical systems and apparatus. These rules are to be considered as addition to Local Authority’s or Company’s regulations.
It is the responsibility of all persons concerned with the operation of and who work on systems and equipment, to make them thoroughly acquainted with these rules.
Repairs work must not be done on any electrical equipment by persons other than properly qualified electricians, unless specially authorized by the Local Management.
All Authorities necessary shall be appropriately informed prior to any work on electrical systems.
15.2 Safety Precautions
Electrical apparatus, plant and equipment shall be made safe prior to any work on it being carried out.
Care shall be taken to ensure that all electrical power supplies, associated with the particular electrical apparatus, plant, or equipment, are isolated and that this done in such a manner to preclude any likelihood of any supplies being accidentally or inadvertently made live. Particular attention shall be paid to the isolation of ancillary supplies for control and indication purposes, etc, and that condensers are discharged in a safe manner.
Circuits shall be shown to be dead by using the appropriate testing equipment. The testing equipment shall be demonstrated to be in working order, both before and after carrying out the tests, and intermediately, if necessary, when testing is prolonged or testing operations are interrupted. When fitted, integral earth equipment shall be used for connecting all main conductors of an isolated unit to earth; alternatively, temporary earths should be fitted in an approved manner.
An approved danger notice card shall be attached to the appropriate point of isolation. No person shall carry out any work of any description, including maintenance; repairs, cleaning and testing of any part of electrical apparatus which are normal live, unless the steps described below have been taken.
• The equipment shall be dead and isolated and all practical steps shall have been taken to lock off the equipment from the live conductors and to post danger notices. Keys shall be retained by the authorized person. Where required the equipment shall be positively grounded by means of circuit breakers, earthling switchers or the like.
• The equipment shall be efficiently earthed as appropriate.
• The authorized person shall be fully conversant with the nature and also the extent of the work to be carried out and with the location and function of any equipment which shall remain liver during the work.
• Where power supplied cannot be isolated at the local position, the equipment shall have been isolated remotely and local positive tests carried out to prove that the circuit is dead.
• Where the design of apparatus precludes the strict compliance with all details of these precautions, the work shall be carried out to the strict instructions of the authorized person present.
• The giving of instructions to make apparatus live, or dead, by signal or prearranged understanding after an agreed interval of time, is FORBIDDEN.
• Whilst it is not always possible to make dead and earth systems, all work on systems shall be carried out as if they were live, unless first proved dead and earthed.
• Guarded and protective items removed during the repair of electrical equipment are to be replaced immediately after the repair work has been completed.
• The wearing of jewellery such as rings, watch bands, metal chains, etc is discouraged. Keep sleeves rolled down when in or close to “live” circuits or apparatus.
• Examine ladders each time before use. Secure or tie ladders into position when necessary. Only non-conductive types of ladders are to be used for electrical work
• Discharge the capacitors before handling or making connections. Examine the safety devices before use to make sure that they are in good working order. Earth all metallic enclosures that have an electrical circuit inside.
• Ensure that a sufficient amount of room is available for safe inspection, repairs or replacements when installing electrical equipment.
• Place yourself in a safe position while working, to avoid falling, stumbling, slipping or moving backward against live parts. Use rubber mats as an additional safety precaution.
• During all work on high to medium high voltage equipment, a minimum of two persons shall present at the work site.
• Prior to work and after completion of it, the safety engineer shall inspect the site in order to make sure that all necessary safety precautions have been taken into account.
15.3 Danger Notices
Clearly visible danger notices shall attached to all switch gear controlling the apparatus which has been made dead and on which the work is preceding e.g. colour tape over a removed fuse. A suitable caution notice shall also be attached to the appropriate circuit breaker or isolator switch on the mimic circuit diagram at the central console.
15.4 Portable Equipment
Portable power tools shall not be used if any defect is suspected or any damaged apparent. Only tools with an armoured cable shall be used. Care shall be taken to avoid damage to flexible cables. Kinking and the formation of knots can cause serious damage which may not be apparent on the outside. Crushing and damage to the outer sheath should be treated as damage necessitating repair.
Only the approved portable battery hand lamps, supplied by the Company, may be used in restricted areas. All hand lamps shall be checked. Battery replacement or repairs shall only be carried out by the Electrical Department and NOT by the user. This shall ensure that certification of approval for use in a restricted area is maintained. When using electrical extension cords, take the following precautions:
• use only approved outlets and connections;
• make sure the cord is properly connected earthed and protected from traffic;
• use care to the cord from becoming wrapped around any part of the body;
• Worn or defective cords must be repaired immediately.
All portable electrical tools must be double isolated. Only approved connections shall be used for joining cables and tools.
16. Mechanical Equipments
16.1 Operation and Maintenance
16.1.1 General
No person may be employed at a machine, plant or equipment unless he has been sufficiently trained in its operation, or unless he works under the supervision of a person who has a thorough knowledge of the operation, the dangers rising in connection with the operation and the precautions to be observed. All crane operators shall be at least 21 years old. Every person, being trained as an operator, shall be fully and carefully instructed as to the dangers arising in connection with such an operation and the precautions to be observed. The use of machinery shall always be in accordance with and within the limits of the manufacturer’s technical specifications.
It is the duty of all employees supervising the operation of machines, plant and equipment, to prevent dangerous practices being performed by persons under their control and to encourage adherence to all reasonable safety precautions.
16.1.2 Precautions
Before starting up any machinery, care shall be taken that no apparatus, tools rags, etc. are in a position to foul a moving part and that no person is in a position of danger. Every machine shall be provided with efficient starting and stopping provisions, so situated as to be conveniently controlled by the machine operator. Lubrication of parts of machinery in motion may only be carried out where it is unnecessary to remove guards. Lubricating oil bottles and grease lubricators shall be checked frequently and any abnormal consumption of lubricant must be reported immediately to the engineer in charge.
Machines fitted with over speed and other trips shall not be operated unless such rips are in full working order. Trips shall be tested at regular intervals in accordance with the manufacturer’s instructions. Machinery shall not be operated with the guards or suitable protection having been removed. Protective clothing and equipment shall be worn as described in the relevant section.
16.2 Lifting Equipment
16.2.1 Local Requirements
All lifting machinery and equipment shall be maintained and operated to comply with the standards of all relevant Local Regulations. A register shall be maintained for all equipment, and details of examinations shall be entered in a register.
16.2.2 Identification Requirements
Capacity plates shall attach to all equipment and shall clearly indicate the safe load and other relevant information for a safe use.
16.2.3 General Lifting Operations
Lifting equipment shall be operated only by personnel familiar with the equipment and with the objects to be lifted (weights).
In case the lifting operation is of a more complex nature, the operator shall only take directions from one responsible person known as the signal man. The operator shall never start the lifting operations until the signal man is clearly within his range of vision or radio communication.
The operator shall ensure that all loads are safely and correctly slung before starting a lift. The safe working load indicator or alarm (if any) shall not be disconnected at anytime when the equipment is in service.
16.2.4 Maintenance
The inspection and maintenance of lifting equipment shall only be carried out by experienced personnel and in accordance with applicable government regulations and the manufacturer’s instructions.
16.3 Hand and Power Tools
16.3.1 Hand Tools
Use the proper tools for the job. The right tool, in good condition and used correctly, seldom causes an injury.
Inspect wrenches and eliminate such defects as worn, broken or sprung jaws, cages, handles and springs. Driving faces of hammers, chisels, drift pins, bars and similar tools shall be free of mushroom heads, breaks and other defects. Wooden handles must be sound and securely wedged or fastened to the tool. Never use a file without a handle. When not in use, sharp edged tools shall be placed in a position that all prevent injury occurrence. Report defective or lack of proper tools to the Supervisor.
Face the jaws of an adjustable wrench in the direction of pull whenever possible. Keep all edged or pointed tools sharp. Avoid using a hammer having a hardened face on a highly tempered tool, such as a drill, file, die, jig, etc.
16.3.2 Portable Power Tools
The majority of accidents caused when using hand tools are due to either improper use of the tool or improper maintenance of the tool. The following precautions should be observed for the following equipment.
16.3.2.1 All Power Tools
All appropriate safety equipment i.e. fuses, guards, etc, shall be lifted and used. All appropriate protective clothing including safety goggles shall be worn. Only competent persons shall operate power tools.
16.3.2.2 Pneumatic Tools
Do not use frayed, damaged or deteriorated hoses for air supply not connects them to air cools. Only use the correct type and size of hose end fittings and connections, use clamps of robust construction especially made for compressed air service.
Shut off the air supply when the air operated tools are not in use. Shut off the air supply and bleed off the pressure before breaking the hose. Do not crimp the hose to shut off the air.
16.3.2.3 Electric Tools and Drills
Tools with cut split or chafed power cables must not be used. Connections between the power lead and the plug must be checked by an electrician. Faulty tools must not be issued.
When using electric drills always use the correct size chuck key; never use wire to secure the chuck key to the power lead; check that the bit is properly located in the chuck, and do not use excessive pressure to speed up the job as this shall only result in a broken bit.
16.3.2.4 Portable Grinders
Inspect the grinding wheel thoroughly for wear & tear. Check if the correct disc is fitted for the job. Dress or change the wheel as appropriate. If the wheel is changed, ensure that the speed rafting of the replacement wheel matches that of the machine. Check the wheel rotation and use it so that the sparks fly away from the body. Use firm but not excessive pressure. Pressure used should not slow the motor. Ensure that the wheel has stopped before putting the grinder down. Disconnect the grinder as soon as possible after use.
16.3.3 Care and Maintenance
Tools not in use shall be returned to stores. The equipment shall be maintained systematically and all defective equipment replaced immediately. All tools shall be inspected for soundness before use. Any worn or damaged tools shall be repaired promptly and soundly. Tools which cannot be repaired shall be discarded.
Tools shall be cleaned after use and returned to the stores or racks. Cutting tools shall be protected and stored in such a manner that they cannot roll or fall off benches or racks.
16. Use of Tools Overhead
Tools hoisted or carried to an overhead platform or scaffold must be placed in a suitable container or tied securely. Tools shall not be carried in an
ordinary belt or pocket; use approved tool belt. Keep both hands free to grasp handrails. Tools used for overhead work must be kept to a minimum number and so placed that they are out of the way and not easily tripped over. Always be alert for the presence of men working below.
16.4 Welding and Burning Operations
16.4.1 General Precautions
Gas and electric welding are safe operations when carried out under correct conditions of work but it must be stressed that the equipment should be properly maintained. Good ventilation and a tidy work area are important factors of safe working. All equipment shall be thoroughly checked before use.
All metals and equipment on which welding, cutting, burning, brazing and soldering work is to be carried out and area in which the work is to be carried out shall be checked to be free of flammable gases, vapours, oil and sludge. Old barrels or drums shall not be used to support work.
No welding, burning, or hot tapping shall be carried out on “live” pipelines except by special permission of the Supervisor. The appropriate protective clothing and equipment must be used.
16.4.2 Preparation and Inspection
An inspection of the area surrounding each location where any burning, welding or other ignition creating activity is expected to be conducted shall be made.
The area surrounding the location of any proposed burning or welding operation shall be examined to ensure that not sparks, flame or hot slag be blown into or fall upon any combustible material or equipment likely to be set on fire or damaged by such sparks, flames or hot slag.
16.4.3 Storage and Handling of Cylinders
Cylinders shall be handling with care. Cylinders shall not be handled roughly, dropped or knocked together. Cylinders shall be secured in an upright position with the caps in place at all times, except when a particular cylinder is being used.
Caps should always be in place on both full and empty cylinders, whenever they are being stored, moved or transported. Never store oxygen cylinders where they may be exposed to oil or grease. Keep sparks, flame and heat away from cylinders. Where possible store the cylinders under cover. Empty gas cylinders must be stored separately from full gas cylinders and shall be treated as full gas cylinders.
Permanent and prominent NO SMOKING signs should be displayed. Always attach the proper regulator and correct colour code hose to the cylinder for which it is designed, before using gas from the cylinder. Note that cylinders from different countries are not always compatible with respect to the fitting required.
Cylinders, valves, regulators and hoses are to be inspected before starting each job. Always keep all threads on oxygen cylinders, regulators and hoses free from oil and grease. Never force connections. Never test for leaks by using a flame or an oily liquid; use soapy water. Never use any leaking equipment. Notify the appropriate Supervisor of any leaks detected. Never tamper with, nor attempt to repair, oxygen and acetylene equipment. Repairs must e done only by personnel. Hoses shall not be laid across any traffic path unless the hoses are protected from potential cutting hazards.
16.4.4 Oxygen and Acetylene
Burning and welding with oxygen and acetylene shall be done by experience and qualified persons only, well versed in the particular safe practices pertaining to their occupation.
16.4.5 Electric Arc Welding
Electric arc welding shall be done by experienced and qualified personnel well versed in the particular safe practices pertaining to their occupation. It is essential that the welding machines, leads and electrode holders to be used are of good design and are maintained in first class operating condition.
All welding cables shall be completely insulated. Cables having worn or damaged spots shall not be used until repaired or replaced. All earth connections shall be securely made to eliminate sparking.
All persons assisting burners or welders should wear appropriate clothing to protect all parts of their body from sparks, hot slag and rays from the welding arc. Always keep the work place clean particularly from stub ends of welding rods, which can roll under the feet and cause a nasty fall? Stand-by personnel shall be assigned outside the confine space and shall be in constant contact with employee (visual of speech) who is working inside. The stand-by Person should have no other duties other than stand-by and shall know to contact in case of emergency. Stand-by personnel shall not enter confined space until help arrives and the only with proper personnel protective equipment such a lifelines and respirators.
B. Rescue efforts shall be well planned and frequently conducted and discussed in regular safety meeting when applicable. Subjects covered should be as follows:
• Person in charge
• Known potential hazard and possible hazard
• Temperature extremes
• Engulfment hazards
• Noise
• Slick and wet surfaces
• Electrical hazards
• Falling objects
• Personal protective equipment needed
Notification and communication to HSE Officer, in the event of any emergency.
NARRATIVE DESCRIPTION OF SITE ORGANISATION The Site Manager
Reports to the Head Office Based on site, he is responsible for:
- From start to completion, responsible for direction of all contract and site related matters
- The implementation of the project QA System
- Ensuring construction is run in a safe and efficient manner

- Training of personnel under his control on established company procedures and good working practice.
The Site Technical Manager
Reports to Site Manager, He is responsible for:
- All engineering matter of the project and commercial management of the subcontractors.
- Review of all engineering documents on receipt and distribution as required (coordination with QA Manager)
- The authority for preparation and verification of purchase orders and subcontract agreements.
- Advising the Project manager in preparation of contractual correspondence.
- Maintenance of the variation & claims register.
- Planning, monitoring and cost control of the Project.
-Design of temporary works
- Co-ordination of all internal and external monthly reporting
- Training of personnel under his control on established company procedures and good working practice
The Construction Manager
Reports to the Technical Manager, He is responsible for:
- Construction of the works in accordance with the Contract specifications, clients
Instructions and the approved drawings, method statements and works instructions.
- Controls all production related activities and staff directly on site.
- Ensures that work is carried out to the quality requirements of the contract.
- Ensures that work is carried out in accordance with the contract programme and highlights any potential shortfalls to the Site Manager
- Responsible for the allocation of the resources within his section to maximize efficiency and outputs.
- Training of personnel under his control on establishes company procedures and good working practice.
The Plant Manager
Reports to Site Manager, He is responsible for:
- Maintenance and repair of all plants, installation of batching plant and all related areas and ensure that they are functioning to the manufacturers instructions and as required by the contract.
- Ordering and control of all plant spares and consumables required.
- Programming and keeping records of all preventive maintenance
- Control and direction of all operators and plant department staff.
- All safety matters related to plant
- Training of plant operators, and keeping records of same
- Maintaining time records of all and hired plant for costing purposes
- Record keeping in accordance with procedures
- Control of rigging, plumbing, mechanical and electrical activities on site.
- Training of personnel under his control on established company procedures and good working practice
The Admin Manager
Reports to Site Manager
He is responsible for:
- Control of all labour related administration matters, including pay, leave, entitlements, welfare, warnings, and terminations.
- Ensuring the company meets the legal requirements related to labour and waste disposal at all time.
- Time keeping and the payroll distribution.
- Setting up and control of the camp, including messing and recreation, consumables and services
- Provision of adequately supplied and manual site and camp medical services
- Control of all transport
- Liaison with the local authorities
- Expediting site purchases in co-ordination with Technical Manager
- Control of site petty cash
- Control and maintenance of site welfare facilities.
- Control of site camp security.
- Training of personnel under his control on established company procedures and good working practice
The Safety Manager
Reports to Site Manager, He is responsible for:
- All site health and safety related matters.
- Notifies Project Manager and Construction Manager immediately of any serious breaches of safety
- All environmental issues
- Accident and near miss reporting
- Production of the site safety plan, policy and procedures
- Risk analysis for all new activities
- Emergency procedures
- Safety training of all site staff in the area
- Training of personnel under his control on established company procedures and good working practice.
